Faith Formation Registration 2024-2025

 Please fill out this form to register your child for Faith Formation beginning the Sunday after Labor Day (Sunday September 8th). 

Any registration fees will be collected ONLY in the Religious Ed Office by Ann Jones, Denise Gupton or Melissa Lane.  Please ensure you have a receipt for any payment you provide.

Sacrament fee: $50 (2nd year only due)

1 child= $65     2 children= $125        3 children= $180        4+ children= $225 max

Any students wishing to receive a sacrament MUST attend Religious Ed for 2 consecutive years and cannot miss more than 5 classes in their 2nd year.
